The following are some of the more general hazard prevention precautions:
Ensure that only the bare minimum tools and equipment are taken into confined space in order to get the job done
Do not enter a confined space unless it is absolutely necessary
Ensure that if the space has a combustible atmosphere that cannot be overcome with purging and ventilation, the proper tools and equipment are taken into the confined space. For example Spark-proof tools
Intrinsically safe tools and “explosion proof” lights and other electrical equipment.
Where moving parts or equipment are within the confined space, ensure all equipment has been locked out at every possible energy source and residual energy has been removed from the system
Where corrosive liquids could be present, ensure that the area has been suitably cleaned or that proper PPE is worn for contact with corrosives
Where gases or liquids could be introduced to the areas, ensure that all supply lines have been closed, valves have been locked in the off position and lines have been blanked where required.
Identify potential sources of contact with electrical energy and ensure that the source is de-energized and tested prior to entry. Strict adherence to lock out/tag out procedure is mandatory
When a space is oxygen-deficient, ensure those personnel entering the space as well as rescue personnel are equipped with self contained breathing apparatus (SCBA or BA for short). The operation and the limits of this equipment must be known, fit testing must be performed, etc. and a sign indicating the area is oxygen-deficient must be posted to warn all personnel
Never ventilate an area with pure oxygen as it increases the fire potential within the confined space
If it is possible that the air quality could change at any time during the entry, ensure that precautions are taken for the routine or continuous air monitoring during the entry
Ensure all welding and other compressed gas lines are removed from the area when not in use in order to prevent a hazardous buildup of gases due to a leak
A pre-entry of the task JSA will help to ensure that you and your co-workers are properly equipped for a safe entry
