Визначення стадії TNM

Т - первинна пухлина

TX — первинну пухлину не можливо оцінити

Т0 — немає свідчень про первинну пухлину

Tis — карцинома in situ: внутрішньопротокова карцинома, лобулярна карцинома in situ, або хвороба Педжета соска без пухлини 1

T1 — пухлина розміром 2см або менше у найбільшому вимірі2

T1 mic — мікроінвазія 0,1 см або менше у найбільшому вимірі

T1а — більша 0.1см, але не більша 0,5 см у найбільшому вимірі

T1b — більша 0,5 см, але не більша 1 см у найбільшому вимірі



T1c — більша 1 см, але не більша 2 см у найбільшому вимірі

T2 — пухлина більша 2 см, але не більша 5 см у найбільшому вимірі

T3 — пухлина більша 5 см у найбільшому вимірі

T4 —пухлина будь-якого розміру з прямим поширенням у грудну стінку3

NX — дистальні метастази не можуть бути оцінені

М0 — немає дистальних метастазів

Примітки:

1.хвороба Педжета, пов'язана з пухлиною, класифікується в залежності від розмірів пухлини
2. Мікроінвазія — це поширення ракових клітин поза мембрану в прилеглі тканини з фокусом не більше 0.1 см у найбільшому вимірі. Якщо є декілька осередків мікроінвазії, використовується розмір найбільшої для класифікації мікроінвазії. (Не використовуйте суму всіх мікроінвазій) Наявність множинних осередків мікроінвазії повинна бути зазначена, як це є з карциномою з більшою інвазією.

3. Стінка грудної клітки включає ребра, міжреберні м'язи і зубчасті передні м'язи, але не м'язи грудної клітки



4. Запальна карцинома молочної залози характеризується дифузним затвердінням шкіри з еризопелоїдним краєм, як правило, без основної маси. Якщо біопсія шкіри негативна і немає локалізованого вимірюваного первинного раку, категорія Т — це рTX при патологічній стадії клінічного запального раку (T4d). Ямочки на шкірі, втягування сосків або інші зміни шкіри, за винятком тих, що є при T4b і T4d можуть бути при T1, T2 або T3, не впливаючи на класифікацію.

Діагностика

Системна терапія (продовж.)

Антрациклін і таксан

Терапія антрациклінами і таксанами

Таксани відіграють активну роль у ад’ювантній терапії,але, незважаючи на те, що було показано, що вони покращують деякі режими на основі адріаміцину, ще немає опублікованих даних, що вони призводять до покращення виживаності порівняно з режимами з антрацикліном

ХІРУРГІЯ

ОПЕРАЦІЯ ЗІ ЗБЕРЕЖЕННЯМ МОЛОЧНОЇ ЗАЛОЗИ ПРОТИ мастектоміЇ

С

Слід заохочувати жінок усвідомити і відчути форму грудей, так що вони знайомі з тим, що є нормальним для них.

а

Антрациклінам слід віддавати перевагу над неантрациклінами у ад'ювантій терапії, оскільки вони дають додаткову користь. Епірубіцин може бути кращим, оскільки він спричиняє менше серцевих побічних ефектів.



А

▪ Всім жінкам з початковими стадіями інвазивного раку молочної залози, які є кандидатами на консерваційну операцію, пропонується вибір між консерваційною операцією (видалення пухлини з чистими полями) і модифікованою радикальною мастектомією.
Вибір операції повинен бути адаптовано до конкретної хворої, яка повинна бути повністю поінформованою про об’єм операції і про те, що опромінення необхідне після операції, якщо поля будуть позитивними.




Слід заохочувати жінок повідомляти про будь-які зміни від норми сімейному лікарю.

а

Таксани слід розглядати у хворих з прогресуючим захворюванням.



С

Консерваційна операція протипоказана, якщо:

▪ Співвідношення розміру пухлини до розміру грудей не вплине на косметичний вигляд



▪ існує є мультифокальне захворювання або велика злоякісний мікрокальцифікат на маммографії
є протипоказання до місцевої радіотерапії ( наприклад, попередня променева терапія на цьому полі, захворюваннях сполучної тканини, серйозні серцеві і легеневі захворювання, вагітність).

Біологічна терапія

А

Психологічна підтримка повинна бути доступною для жінок з діагнозом раку молочної залози в клініці

С

Трастузумаб повинен бути зарезервований для тих пацієнток, у яких пухлина має надмірну експресію HER2 .

С

Можливість реконструкції молочної залози повинна обговорюватися з усіма пацієнтками до мастектомії.



Направлення від первинного до спеціалізованого фахівця повинно бути у відповідності до настанови з раку молочної залози Шотландії

а

Комбінована терапія з трастузумабом і таксаном рекомендується у жінок з метастатичним раком молочної залози, оскільки вона асоціюється з кращою виживаністю в порівнянні з терапію таксаном







Дослідження

Бісфосфонати

хірургічне лікування пахви

Всі пацієнтки повинні мати повне клінічне обстеження.

А

Бісфосфонати повинні постійно використовуватися в комбінації з іншою системною терапією у пацієнтів з метастатичним раком молочної залози з метастазами у кістки. Вибір препарата для конкретної хворої залежить від індивідуальних обставин.

А

Пахвова операція повинна виконуватися в усіх групах хворих з інвазивним раком молочної залози

Гормонотерапія

Лікування протокової карциноми in situ

В

Там, де локалізована аномалія пацієнтки повинні проходити діагностичне обстеження методами візуалізації з наступною аспіраційною біопсією тонкою голкою для цитології або біопсією тканини.

А

Передменопаузальні жінки, чиї пухлини не мають рецепторів естрогену і прогестерону, повинні розглядатися на отримання ад'ювантної гормональної терапії

B

Жінкам з протоковою карциномою in situ, які є кандидатами на операцію повинен бути запропонований вибір лампектомії або мастектомії

В

Ураження вважається злоякісним після клінічного огляду, знімків або цитології, де це можливо, гістопатологічного підтвердження злоякісного процесу до будь-якої хірургічної процедури (наприклад, мастектомії або операції на пахві).

А

▪У жінок в постменопаузі з раком молочної залози тамоксифен залишається препаратом вибору в якості початкової терапії в ад'ювантному режимі. За наявності відносних протипоказань до
його використання (високий ризик тромбоемболії або порушення з боку ендометрія) або непереносимості, інгібітори ароматази можуть бути використані замість нього.

▪ Постменопаузальні жінки повинні розгляндатися на предмет переходу на інгібітори ароматази після двох-трьох років або через п'ять років терапії тамоксифеном.


▪ У жінок в постменопаузі з поширеним захворюванням інгібітори ароматази третього покоління слід розглядати перед тамоксифеном або мегестрол ацетатом.

Жінки, у яких виконана консерваційна операція, пропонується післяопераційне опромінення молочної залози.

Пацієнти повинні обстежуватися в міждисциплінарний клініці за участю торакальних лікарів, радіологів і цитологів.



А



Переваги і недоліки гормональної терапії повинні обговорюватися з жінками з протоковою карциномі in situ і вибір лікування приймається на основі індивідуальних обставин


терміни проведення хірургія і хіміотерапія

У пацієнтів з симптомами хвороб два види маммографії повинно бути виконано в рамках потрійної оцінки (клінічна оцінка, обробка зображень та відбір проб тканини) в спеціалізованій клініці




При поширеному захворюванні комбінація тамоксифену плюс видалення яєчників повинно бути запропоноване перед тамоксифен терапію

С

Всі процедури для пацієнтів з ранніми стадіями раку грудей слід починати, як тільки це буде можливо. Молоді жінки з рецепторами естрогену негативні пухлини можуть скористатися особливо з раннього початку хіміотерапії після операції.

Лікування симптомів менопаузи

Маммографія не рекомендується жінкам у віці до 35 років, якщо є сильна підозра раку







В

Мегестрол ацетат або депо внутрішньом'язового медроксипрогестерон ацетату може розглядатися для контролю тяжкості приливів у жінок з раком молочної залози.

Магнітно-резонансна томографія повинні розглядатися в конкретних клінічних ситуаціях, коли інші методи візуалізації не є надійними, або були безрезультатними, а де є ознаки того, що МРТ є результативною













СИСТЕМНА ТЕРАПІЯ

Ад’ювантна хіміотерапія


Визначення психологічних порушень

Психологічна підтримка жінок з раком молочної залози і їх сімей

А

Всіх жінок у віці до 70 років, з ранніми стадіями раку молочної залози слід розглядати на ад'ювантну хіміотерапію.

В

Лікування психологічних симптомів у хворих на рак молочної залози повинно здійснюватися відповідно до індивідуальних обставин хворих ( наприклад, високий рівень психологічних порушень або ризик виникнення фактрів, які можуть спричинити проблеми)

А

Групи психологічного втручання повинні бути доступні для жінок з раком молочної залози, які відчувають потребу в їх допомозі. Було показано, що підтримуюча терапія є ефективною при поширеному захворюванні, а когнітивно-поведінкова терапія при локалізованому, локально-регіонарному або поширеному захворюванні.

Жінки з ЕР-позитивними пухлинами, які отримують хіміотерапію повинні розглядатися на отримання гормональної терапії, особливо якщо вони молодші 35 років

В

опитувальники, які застосовуються рутино, не слід застосовувати для виявлення клінічно важливих психологічних симптомів у хворих на рак молочної залози, у яких немає факторів ризику сильної тривоги або психологічних порушень.

А

когнітивно-поведінкова терапія (групова або індивідуальна залежно від преференцій і наявності) пропонується деяким хворим з тривогою і депресією.









Служби з захворювань молочної залози повинні рутинно здійснювати скринінг на наявність психологічних порушень або факторів ризику значних психологічних порушень з моменту встановлення діагнозу (включно під час контрольного спостереження)

Мультидисциплінарні команди повинні узгодити протоколи для оцінки психологічних порушень і їх лікування. Вони повинні включати рекомендації щодо направлень і шляхів надання медичної допомоги

Телефонні і комп’ютерні втручання слід завжди пропонувати хворим

ПРОМЕНЕВА ТЕРАПІЯ

Ад’ювантна променева терапія

КОМУНІКАЦІЙНІ МЕТОДИ

Променева терапія повинна призначатися після мастектомії або операції зі збереженням груді для зменшення локальних рецидивів у осіб, де користь перевищує ризики ускладнень опромінення


А

Хворим на рак молочної залози слід пропонувати аудіо касети або листівки з резюме важливих консультацій.

Клінічні зустрічі з хворими на рак молочної залози повинні полегшувати вибір хворої щодо рішень про лікування ( мається на увазі залучення хворих до процесу прийняття рішення)

Плани в письмовому вигляді, засоби для негайного прийняття рішень повинні використовуватися для поліпшення спілкування з хворими на рак молочної залози

Клініцистів слід заохочувати проходити навчання з навичок спілкування

D

Надключична зона повинна опромінюватися у всіх пацієнток з чотирма або більше позитивними пахвовими лімфатичними вузлами

Психологічна допомога

Роль медичної сестри з молочної залози

спостереження і паліативна допомога

Усі жінки з потенціальним або відомим діагнозом раку молочної залози повинні мати доступ до медичної сестри спеціаліста з молочної залози для отримання інформації та підтримки на кожній стадії діагнозу і лікування.

Маммографія повинна використовуватися для виявлення рецидивів у хворих, які пройшли лікування з приводу раку молочної залози



Деталі контактів і інформація про роль медичної сестри з раку молочної залози повинні бути доступні пацієнткам, їх сім’ям і членам їх міждисциплінарної команди, включно команду первинної медичної допомоги
